Individual plan: $199
Family plan: $149
$149/ for each additional member
Benefits include:
2 regular checkup exams per calendar year.
1 set of checkup X-rays per calendar year.
2 regular cleaning ( if different type of cleaning was recommended during exam, fees may apply).
1 emergency exam per calendar year.
20% off most of other dental services. ( No maximum, no restrictions).
